Most of the turmeric people take for inflammation never gets past the gut. That single fact explains almost everything confusing about curcumin: why the laboratory results look spectacular, why so many human trials came back with nothing, and why the trials that did work mostly used a form of curcumin you cannot buy in a spice jar. I have spent a lot of my working life reading this research, and a fair bit of my personal life taking it, so here is the honest version.

Quick answer

  • Curcumin is the active compound in turmeric. Turmeric powder is only about 3% curcumin by weight, and that curcumin is very poorly absorbed.
  • Human trials of curcumin now cover inflammatory bowel disease, rheumatoid arthritis, psoriasis, male fertility and several other autoimmune conditions. Many found benefit. Some found none, and the failures cluster around plain, low-absorption curcumin.
  • The form matters more than the dose. Absorption-enhanced forms (piperine combinations, phytosomes, nanomicelles, solid lipid particles) are what the positive trials used.
  • Curcumin is well tolerated in trials, but it interacts with blood thinners and some cancer treatments. Tell your doctor or pharmacist before starting.

Turmeric or curcumin, and does it matter?

Turmeric is the root. Curcumin (strictly, a family of curcuminoids) is the yellow pigment inside it, and it is the part that has been studied for inflammation. By weight, turmeric is roughly 3% curcumin, so a heaped teaspoon of the spice gives you under 100 mg of curcumin [1]. No clinical trial in any condition has used culinary turmeric, and the golden latte is a pleasant drink rather than a treatment.

Curcumin capsules are the next step up, and a lot of shops sell them. The problem is what happens after you swallow one.

The absorption problem

Curcumin barely dissolves in water, the liver and gut wall break it down quickly, and what does get into the blood is cleared within hours [2]. Give a healthy volunteer a large dose of plain curcumin and blood levels stay close to zero. Researchers noticed this early. A 1998 study found that taking curcumin with piperine, the compound that makes black pepper hot, increased absorption in people by about 2,000% [3]. That paper started twenty years of work on formulations built to get curcumin into the body.

There is one useful exception to the absorption story: the gut itself. Curcumin does not need to reach the bloodstream to reach the lining of the colon. Several fascinating studies actually measured curcumin inside human colon tissue after people took it by mouth, at levels the researchers called "pharmacologically active" [4, 5]. In one, curcumin in the colon lining reduced a marker of DNA damage by up to 58% [4]. A nano formulation put around five times more curcumin into that tissue than the standard form [6].

This is the key to reading the trial literature. Where the target is the gut, plain curcumin has a chance. Everywhere else, the form decides whether the trial could have worked at all.

What the human trials have found

Crohn's disease and ulcerative colitis

The most complete evidence base. Eighteen human studies since 2005, most positive, with the strongest result from a 12-week placebo-controlled trial of a nano-formulated curcumin in active Crohn's, where 40% of people reached clinical remission against 0% on placebo [7]. UK dietetic guidance (2023) and the European Crohn's and Colitis Organisation (2025) now list curcumin as an option alongside standard treatment. Rheumatoid arthritis

A 2012 pilot trial found curcumin at least matched a standard anti-inflammatory drug on disease-activity scores, with no drop-outs for side effects [8]. A 2019 placebo-controlled trial of a curcumin nanomicelle reported reduced joint pain, swelling and morning stiffness [9], and a 2023 phase III trial tested curcumin for keeping people in remission while their disease-modifying drugs were withdrawn over 52 weeks [10]. A 2022 review pooling 31 randomised trials across ten autoimmune diseases concluded curcumin "had good clinical efficacy" in rheumatoid arthritis [11]. Psoriasis

Trials here mostly add curcumin to existing treatment. A 2015 study found steroid cream plus an absorption-enhanced curcumin cleared plaques further than steroid cream alone, with an inflammatory marker (IL-22) falling only in the curcumin group [12]. A 2018 trial found acitretin plus nanocurcumin outperformed acitretin alone in moderate-to-severe disease [13]. The same 2022 review rated the psoriasis evidence as good [11]. Male fertility

One randomised, double-blind, placebo-controlled trial in 60 men with poor semen quality gave 80 mg of curcumin nanomicelle daily for 10 weeks and reported higher sperm count, concentration and motility, with lower markers of oxidative stress and inflammation [14]. It is a single trial, and I say so in the full article, but it is a well-designed one. Full article: Curcumin and male fertility.

Autoimmune disease more broadly

The 2022 review covered ten conditions and found enough trials to draw conclusions in three (psoriasis, ulcerative colitis, rheumatoid arthritis). For lupus, Hashimoto's thyroiditis, lichen planus, Behçet's and others the trials are small and few. That is a comment on the amount of research, not a verdict against curcumin, and I go through each condition in Curcumin and autoimmune disease.

The forms of curcumin, compared

A note on doses. Plain curcumin trials used 1 to 3 g a day. The nanomicelle trials used 80 mg two or three times a day, a tiny fraction of that, because so much more of it is absorbed. Comparing milligrams across forms tells you nothing.

Dose and safety

Curcumin has a long safety record in trials, at doses up to several grams a day of the plain form, with digestive upset the most common complaint [16]. The international acceptable daily intake is 3 mg per kg of body weight for plain curcumin, which for an 11-stone adult is about 210 mg [17].

Three cautions matter:

  • Blood thinners. Curcumin has a mild antiplatelet effect and is best discussed with your doctor if you take warfarin, apixaban or similar.
  • Cancer treatment. Some chemotherapy agents interact with curcumin. Anyone in treatment should ask their oncology team first.
  • Pregnancy. Culinary amounts are fine. Supplement doses have not been studied in pregnancy, so the usual advice is to avoid them.

If you have gallstones or a bile duct problem, ask before taking it, since curcumin stimulates bile flow.

Frequently asked questions

Is curcumin the same as turmeric?
No. Curcumin is the active compound inside turmeric, about 3% of the spice by weight. Supplements are standardised curcumin extract, sometimes in a form built for absorption.

Does black pepper make curcumin work?
It makes more of it absorb, around 20 times more in the classic study [3]. It does not change what curcumin does once absorbed, and piperine can raise blood levels of some medicines.

How much curcumin should I take for inflammation?
It depends entirely on the form. Plain extract trials used 1 to 3 g a day. Nanocurcumin trials used 80 mg two or three times a day. Follow the label of the form you have.

How long does curcumin take to work?
In the trials that reported timing, changes appeared between four and twelve weeks. Nothing in the literature supports an overnight effect.

Can I take curcumin with my medication?
In the IBD and arthritis trials it was taken alongside standard drugs, including biologics, without extra side effects. The exceptions are blood thinners and some cancer treatments. Ask.

Is nanocurcumin better than ordinary curcumin?
For getting curcumin into the body, yes, and the trials that used it are the strongest we have. Whether it is better for you depends on what you are taking it for, which is why the condition articles above exist.

One capsule a day is a three-month supply per bottle. I formulated it because the research I keep citing was done with absorption-enhanced curcumin, and the plain capsules on most shelves are not that.
