What are varicocele?
Varicocele is one of the most common and treatable causes of male infertility, affecting up to 15% of all men and up to 40% of men with primary infertility.
Varicocele increases testicular temperature and significantly elevates oxidative stress within the testes. This impairs sperm DNA integrity and may negatively affect testosterone production.
While varicocelectomy remains the gold-standard treatment when clinically indicated, not all men require surgery — and some prefer conservative (non surgical) approaches. Although surgery can reduce oxidative stress and inflammation, these processes may persist even after a successful procedure. This is where targeted nutritional strategies become clinically relevant.
The following nutrients have been investigated in human studies and shown potential to improve outcomes in men with varicocele:
Best Evidence-Based Supplements for Varicocele and Male Fertility: Antioxidants
Multiple human studies show that men with varicocele have significantly elevated seminal reactive oxygen species (ROS) and higher rates of sperm DNA fragmentation. The following nutrients have been studied in men with varicocele and combat excess oxidative stress, reducing lipid peroxidation and sperm DNA fragmentation in men with varicocele
1. Alpha Lipoic acid at a dose of 600mg per day
2. N acetyl cysteine (NAC) at a dose of 600mg per day
3. Selenium at a dose of 200mcg per day. A 2011 study reported that men with varicocele had lower selenium concentrations on their semen compared to normozoospermic men ((men with normal sperm parameters). Lower selenium levels were associated with reduced sperm concentration, motility and morphology, whereas higher levels correlated with improved semen parameters.
4. Zinc at a dose of 10mg per day
5. Vitamin C – 100mg or more per day
6. Vitamin E – 15mg or more per day
Best Evidence-Based Supplements for Varicocele and Male Fertility: Mitochondrial support
Sperm motility is heavily dependent on mitochondrial ATP production in all men but especially in men with varicocele
7. Acetyl-L-carnitine at a dose of 1,000mg per day
8. Coenzyme Q10 at a dose of 100mg per day
9. L-carnitine at a dose of 1000mg per day

10. Vitamin B2 (Riboflavin)
11. Vitamin B6 (Pyridoxine)
12. Vitamin B9 (Folate or folic acid) – interestingly, in one study, polymorphisms in the MTHFR gene were associated with 230% greater chance of varicocele. Further, many research studies have reported benefits of Folate and B-vitamins.
13. Vitamin B12 (Methylcobalamin)

14. Probiotics – some probiotic strains have beneficial effects of male fertility, sperm quality and even varicocele.
Other nutrients, supplements & vitamins for male fertility and varicocele
A 2016 study measured omega 3 levels in the sperm of men with infertility and varicocele. The researchers reported that the less omega 3 in the sperm, the greater the degree of varicocele and DNA damage. Although this study did not provide omega 3 supplementation, the researchers wrote: ‘The findings suggest that omega-3 PUFA deficiency could be implicated in varicocele-associated infertility’.
Omega 3 fatty acids may improve outcomes in men with varicocele by
· Reducing inflammatory cytokines
· Improving membrane fluidity
· Reducing sperm DNA fragmentation
This research once again highlights the important role of omega 3 for male fertility and sperm quality. Omega 3 can be consumed in the diet via oily fish such as salmon, sardines, mackerel or ground flaxseed/linseed, walnuts or chia seeds daily. It is also worth thinking about a high-quality, purified omega-3 supplement such as Omega 3 Phix if you have varicocele.
Collectively, these 14 nutrients and omega 3 target the key physiological drivers of varicocele infertility:
· Inflammation,
· Oxidative stress,
· Mitochondrial dysfunction,
· Hormonal imbalance.
These nutrients represent promising adjunctive strategies alongside medical care making them best supplements and nutritional strategies for men with varicocele.
Men with varicocele trying to conceive should consider targeted inflammation and oxidative stress support including supplementing these 15 nutrients.
Implementing these nutrients individually can be complex, requiring multiple supplements and careful dosing.
Fertility Phix is an all in one fertility supplement and was formulated to provide these research-backed nutrients in evidence-informed doses, alongside additional compounds that support overall male fertility and sperm quality. Fertility Phix has already helped many men with varicocele to improve their sperm quality and ultimately helped the partners of many men with varicocele to become pregnant – check out these reviews:
What Does the Clinical Research Say?
Randomised controlled trials have demonstrated improvements in sperm concentration, motility and morphology following specific antioxidant therapy in men with varicocele. Meta-analyses also suggest that antioxidant combinations may reduce sperm DNA fragmentation and improve pregnancy rates.
Who Benefits Most?
Men who may benefit most include:
- Men with varicocele and abnormal semen parameters
- Men who need to or want to avoid surgery
- Men after varicocelectomy
- Men with elevated DNA fragmentation index (DFI)
- Men with elevated oxidative stress markers
- Men with suboptimal testosterone
Key Takeaways
· Varicocele increases oxidative stress and impairs sperm DNA.
· Surgery remains gold standard when clinically indicated.
· Targeted nutritional support may improve semen parameters.
· A minimum of 3 months is typically required to see meaningful changes in semen parameters.
· Work with a clinician

· Not all supplements are appropriate for everyone. Always consult your GP or fertility specialist before starting supplements, particularly if undergoing assisted reproductive treatment.
· Timing matters – lifestyle changes, including supplements are recommended for at least 3 months before conception. This is because a man’s sperm takes approximately 2 to 3 to 4 months (roughly 75 days) to mature, in a process known as spermatogenesis.
· Dosing matters – scientific research has reported benefits of certain doses. It is important to seek out and consume these doses to get the same benefits.
If you do decide to try Fertility Phix, we recommend not taking other supplements at the same time. I, Conor, formulated Fertility Phix to be as comprehensive as possible – meaning you do not need to take additional supplements. Further, some additional supplements could even cause harm such as additional supplements containing vitamin D, selenium or iodine for example because Fertility Phix already contains these. If you want to take Fertility Phix and another supplement, work with your healthcare provider(s) and/or contact me, Conor, here at Phytaphix.
